Questions to ask about insurance
Do not assume an in-network copay applies at Bloom. Before deciding how to pay, ask your insurer whether your plan has out-of-network outpatient mental health benefits and what documentation it needs.
- Does my plan include out-of-network outpatient mental health benefits?
- Is there a deductible, and how much have I met?
- How is reimbursement calculated for the actual service code the practice will bill?
- What claim documents and submission deadlines apply?
Ask Bloom about the documentation it can provide and use your insurer’s answer to understand your own benefits. Reimbursement depends on your plan. Paying for treatment and receiving an insurance reimbursement are separate steps.
